Kōzenji Station opened on 15 December 1910 between today's Kōrien and Hirakata-kōen on the newly built Keihan Main Line. Tracks east and west of the station originally ran in city streets but were relocated onto private right-of-way in July 1931. The Imperial Japanese Army's Kinya magazine explosion of 1 March 1939 damaged the line, suspending service here until 6 March. Following the 1943 merger Kōzenji belonged briefly to Keihanshin Express, returning to Keihan Electric Railway on 1 December 1949. The current overhead station building opened on 29 November 1998, and Keihan plans to relocate the platforms onto an elevated alignment by 2028.

Although both neighbouring stops are express stations, Kōzenji's weekday boarding numbers exceed those at Hirakata-kōen because multiple high schools cluster around the station.
